Nursing Services

NDNS offers a wide range of services to help you remain independently in your own home.  Our services are provided by a professional team of nurses and carers.Services include post hospitalisation care, wound care, falls prevention, rehabilitation, dementia care, management of medications and incontinence, palliative care, disability care and support, grief and bereavement support and a broad range of other technical nursing care.
 

 

 


 

Care Co-ordination is provided by a Registered Nurse or Cert IV who works in consultation with the client / carer and family. The Co-ordinator uses a collaborative approach for: assessing an individual’s needs; holistic care planning; coordinating multiple services; and regularly reviewing the client’s wellbeing. This assists clients with complex care needs to live as independently as possible in their own home.

Our in-home respite services are available for carers who need a break from their caring role. NDNS will temporarily substitute the role of the carer and deliver a flexible range of services, including domestic assistance, shopping, personal care and medication monitoring.


 

Services includes assistance with personal hygiene, showering, dressing, grooming, and other personal support needs such as transferring into and out of bed.




 

Clinical nursing is provided by one of our Registered Nurses in your home. Services include post hospitalisation care, initial assessments, professional reports, hygiene and infection control, review of nursing management, nutrition and wound healing, wound care, falls prevention, rehabilitation, dementia care, management of medications and incontinence, palliative care, grief and bereavement support.

Case Management is provided by a Registered Nurse who works in consultation with the client / carer and family. The Case Manager uses a collaborative approach for: assessing an individual’s needs; holistic care planning; coordinating multiple services; and regularly reviewing the client’s wellbeing. This assists clients with complex care needs to live as independently as possible in their own home.

NDNS has a Dementia Specific Care Team whom work together to provide clients and their families carer support, respite options, advocacy, long term supports and future planning.
